REMINDER: Parents of students that qualified for free or reduced meals by direct certification through DHS, should have received a letter in the mail. On the back of the letter was a fee waiver request that parents need to sign and return to the school. You can either bring it to one of the building secretaries or mail it to Linda Beyer, PCM Middle School, PO Box 490, Prairie City, IA 50228. Just because you qualify for free or reduced meals does not mean you qualify for a fee waiver. The waiver request must be signed and returned to the school. Parents: Reminder if your student is participating in PCM Middle School Sports and current physical, concussion form and insurance waiver are required. Physicals are kept on file until they expire (1 year and 1 month from the date of physical). You must complete a new concussion form and insurance waiver each year. Forms are available on-line or in the buildings.
